The documents must be printed and signed by both the participant and one of the participant's parents or legal guardians if the participant is a youth. Please make sure to sign both forms. All participants must complete the forms, including Ma’s, Pa’s, and anyone who will be assisting on the Trek. We also ask that your firmly attach a recent photo of yourself to the photo release. These photos will not be returned and will be used to assist the ma’s and pa’s in getting to know their families before the Trek begins.
There is a Ma and Pa from each ward that has been designated as Ward Trek Coordinators for your ward. Please turn in your completed registration packet to them and they will turn them in to the stake.
